Job Description

We are looking for an enthusiastic and motivated Clinical Pharmacist to join our friendly and forward-thinking General Practice
As part of our multidisciplinary clinical team, you will play a key role in supporting safe, effective, and evidence-based prescribing, improving patient outcomes, and reducing the workload of GPs through expert medicines management and clinical care.
You will work directly with GPs, practice nurses, pharmacy technicians, and other healthcare professionals to optimise medicines use across the practice population. Your work will include structured medication reviews (SMRs), chronic disease management, repeat prescription reviews, medicines reconciliation, and supporting long-term condition pathways in line with local and national priorities.
If you are an experienced pharmacist andIndependent Prescriber, this is an excellent opportunity to develop your clinical skills in a supportive, innovative environment. You will be encouraged to undertake further training CPPE Primary Care Pharmacy Education Pathway (PCPEP)) and to contribute to quality improvement and service development .
Main duties of the job
Conduct and document structured medication reviews (SMRs).
Provide face-to-face and telephone consultations for medication queries and chronic disease management.
Support prescribing safety, cost-effectiveness, and adherence to local formularies and NICE guidance.
Lead or support medicines optimisation projects, audits, and prescribing initiatives.
Collaborate closely with community pharmacies, secondary care, and other local health partners.
Work within the NHS England Additional Roles Reimbursement Scheme (ARRS) framework.

  • Independent Prescriber (IP) Qualification
  • Independent Prescriber (IP) qualification recognised by the GPhC.
