Main duties of the job
There is a focus to develop the team and our medicines management services further with more patient facing clinics and pharmacist-led management of chronic disease, medicines optimisation and managing polypharmacy and appropriate pragmatic prescribing in the elderly amongst other initiatives. Guidance and support for the pharmacy and medicines management team to deal with prescriptions/medicine/prescribing queries is core. The role has a high degree of autonomy.
To work with the GP prescribing lead in the practice and support with achieving efficient and effective prescribing activities.
Working as part of a multi-disciplinary team in a patient-facing role, to provide primary support to general practice staff with regards to prescription and medication queries. Ensuring that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes.
