Working hours: 17 hours over 2 full days - Monday & Friday (we can be somewhat flexible on this)

Job Purpose

The Prescribing Paramedic Practitioner will work as part of the multidisciplinary team (MDT) to provide advanced clinical care, including assessment, diagnosis, treatment, prescribing, and onward referral of patients within a primary care setting. The role supports both planned and same-day urgent care, delivering high-quality, patient-centred services that reduce demand for GP appointments and improve access for patients.

Key Responsibilities

Undertake triage and clinical assessment of patients, including urgent, same-day, and routine presentations. Provide autonomous, evidence-based clinical care and treatment for patients, including prescribing medications in line with national/local guidelines and care pathways. Support home visits and provide an alternative model for GP urgent home assessments. Perform and interpret ECGs, blood pressure readings, spirometry (where trained), and other diagnostic tools. Undertake investigatory procedures and collect specimens including blood, swabs, urine samples, etc., within scope of practice. Request and interpret appropriate investigations to support clinical decision-making. Independently prescribe medications in accordance with local formulary, guidelines, and own scope of competence. Provide medication reviews and advice to patients on medicine use, interactions, and lifestyle considerations. Offer lifestyle advice and promote self-care where appropriate, including signposting to social prescribing and community services. Participate in delivery of health checks, chronic disease reviews, and anticipatory care planning (e.g., for frail or elderly patients). Work collaboratively with the MDT, including GPs, nurses, clinical pharmacists, care coordinators, and other allied health professionals. Attend and contribute to team meetings, case reviews, and safeguarding discussions. Communicate effectively with patients, carers, and external agencies, including handling sensitive or complex information. Maintain accurate, contemporaneous, and comprehensive clinical records. Ensure completion of necessary documentation related to consultations, referrals, investigations, and care plans. Adhere to information governance, safeguarding, health & safety, and confidentiality policies.
Person Specification

Essential

Degree in Paramedicine or equivalent qualification Registered with the Health and Care Professions Council (HCPC) as a paramedic Independent Prescribing qualification (V300) Evidence of ongoing professional development Working towards or attained Level 7 (MSc) capability in clinical practice Experience working in a primary care, urgent care, or general practice setting Strong IT skills Knowledge and understanding of confidentiality, safeguarding, health and safety, and information governance policies
