Job Description

Practice Plus Group is seeking an experienced Prescribing Pharmacist to join our Devon Integrated Urgent Care (IUC) and Clinical Assessment Service.
In this role, you will provide pharmacy leadership and support within the Clinical Hub, working as part of a dynamic multi-disciplinary team to deliver high-quality, evidence-based care that meets the needs of patients across Devon.
You will play an active role within a cross-disciplinary, multi-skilled team, helping to ensure service quality aligns with corporate goals and strategy. You will also contribute to the development of innovative models of care that positively impact the wider health economy.
Working closely with the Regional Manager, Pharmacy Lead, Medical Lead, and the wider clinical team, you will help ensure the service consistently meets its contractual and clinical obligations.
About you
You will be a qualified and registered Pharmacist (BSc or Master's Degree in Pharmacy) with current registration with the General Pharmaceutical Council (GPhC). You'll bring a genuine passion for developing your own pharmaceutical expertise as well as supporting the growth of your colleagues.
You will have experience in leading and managing teams, with a strong focus on continuous improvement of processes and procedures. You'll be flexible, collaborative, and open to new ideas, with the confidence to work autonomously and show initiative when managing a wide range of clinical situations.
This is a zero-hours contract, offering flexibility to choose sessions that suit you, available on either a bank or self-employed basis.
What you'll be doing
Your responsibilities will include:
Providing safe and effective telephone advice on medicines use and pharmaceutical care to clinicians, patients, and carers
Prescribing appropriately, including repeat medications, in line with agreed service parameters
Offering clear self-care and medicines management advice
Managing and implementing medication changes following hospital discharge or outpatient appointments
Assessing and managing requests relating to:
Repeat prescriptions
Medication queries
Health information
Minor ailments and onward referral
Prescribing in accordance with Practice Plus Group policies and local and national clinical guidelines
Signposting patients to appropriate services where required
Working collaboratively within the multidisciplinary team, adhering to agreed roles, responsibilities, and management guidelines, including those relating to Non-Prescribing Medics (NPM)
What we'll look for in you
You will be a GPhC-registered Prescribing Pharmacist with strong clinical judgement and experience in medicines management, including repeat prescribing and post-discharge medication changes. You'll be confident providing telephone-based pharmaceutical advice and prescribing in line with Practice Plus Group policies and local and national guidelines.
You'll take a patient-centred approach, offering clear self-care advice and knowing when to signpost patients appropriately. Strong communication skills and the ability to work both autonomously and collaboratively within a multidisciplinary team are essential, along with a commitment to quality, safety, and governance.

Prior to joining Practice Plus Group I spent almost 10 years working in community, but I have found that the prison environment is where I feel I can utilise my skills and truly make a difference. I see patients with complex health issues, I help keep them informed about their medication and support out-patients in managing their conditions day to day.
