The Directorate will build and maintain a broad range of relationships to further its aims and will connect ICB and ICS Strategy with local delivery - emphasising and valuing hyperlocal knowledge and clinical/ professional contributions. The Directorate is dynamic and flexible and will continue to evolve as the ICB and ICS matures.

The Medicines Optimisation Team is under a unified 'one team' leadership structure for NCL, organised around three specialisms:

Medicines Planning and Operations; local teams with a hyper local operational focus, working as 'one team' with flexible resources for deployment across the five boroughs. Medicines Quality and Improvement; an NCL wide function, delivering activities 'once for NCL'. Medicines Strategy and Programmes; an NCL wide function focused on commissioning contracting, transformational programmes and analytics.
This role will contribute to the delivery of:

an operating model for NCL Medicines Optimisation. major programmes across primary care, shaped by national or local medicine optimisation priorities. operate at the relationship interface with practices etc, and supporting the work as required to transition stakeholders to a set of shared expectations.
The post will primarily support the Planning and Operations team, but in line with the 'one team' approach may be required to work with/ support the other specialisms within Medicines Optimisation: Quality and Improvement, or Strategy and Programmes.

Contribute to a Medicines Optimisation operating model that:

? provides proactive support to primary care across prescribing, workforce and service development.

? identifies and addresses interface issue management e.g. discharge.

? maintains and balances local knowledge, insight (including soft intelligence), visibility and relationships with operational flexibility.

? dedicates capacity on driving adherence to core quality and safety standards.

? enables prescribing hotspots and unwarranted variation at neighbourhood, place and system to be addressed in a timely and effective way.

As directed by the Prescribing Advisor, Senior Prescribing Advisor and/ or Head contribute to designated and defined areas of medicines optimisation work. Promote and maximise rational, safe and cost-effective prescribing by prescribers (medical and non-medical) in the ICB, within available resources and balance this with improving and maintaining the quality of prescribing in line with therapeutic developments. Provide hands on support and advice on matters relating to the use and supply of medicines, particularly to and working with GP practices, in accordance with national and regional policy. including, to:
? Conduct audits related to prescribing and appropriate medicines use, and work with practice to implement best practice.

? Support for PCNs to be able to provide clinical medication review and lifestyle advice for identified patients using all levels of medication review.

? Discuss outcomes of reviews and proposed recommendations to improve quality of

prescribing and cost effectiveness with clinicians at relevant meetings.

Facilitate the delivery of ICB prescribing and medicines optimisation strategies at practice and individual patient level. Ensure clinical governance through good record keeping, patient confidentiality and incident reporting. Adhere to risk management systems for the prescribing, preparation, supply, storage, transport of medicines and that systems are in place to encourage reporting of adverse events including medication errors. Ensure that errors in relation to the use of medicines are reported and investigated in line with ICB incident reporting systems. Contribute to appropriate budgetary controls and the annual CIP planning and delivery process, working closely with colleagues and providers to influence prescribing behaviours in general practice, community pharmacy and more broadly. Understand the ICB vision and strategic priorities (population health, tackling inequalities) and work with colleagues to deliver this for and across the medicines optimisation function. Contribute to the design/ planning/ implementation/ evaluation of major programmes of work e.g. IPMO, CIP shaped by national or local priorities, working closely with peers and Heads of in Medicines Optimisation. Implement programmes that are rooted in evidence based practice, support innovation and address clinical quality and sustainability issues in addition to being financially sound. Contribute to the implementation of critical enablers for medicines optimisation; digital, data analysis, workforce and training. Support medicines optimisation governance as directed by the Prescribing Advisor, Senior Prescribing Advisor and/ or Head of e.g. development of Committee papers, policies, drafting of business cases, input into contract documentation. Facilitate good, collaborative relationships with all internal/ external stakeholders; to support the ICB medicines optimisation function to be at the heart of opportunities to integrate care, narrow inequalities and improve population health. Work/ interface with partners to implement the system vision for medicines optimisation through clinical and patient engagement and linking national medicines policy and local clinical priorities. Support the dissemination of medicines optimisation communications; GP and ICB website. Contribute to the drafting of ICB corporate responses to medicines optimisation requests (Freedom of Information, complaints, patient enquiries), in line with ICB policy and standard operating procedures.

What you'll need:



A minimum of 6 months experience

Prescribing Support or similar role

within the last 3 years preferably with an ICB

Educated to masters degree pluspre-registration training and experience.

Pharmacist registered with the General Pharmaceutical Council (GPC).

A good understanding of the NHS (System/ Place/ Neighbourhood)

and the wider public sector.

Excellent knowledge of project/programme management (principles/ techniques/ tools), service delivery, business processes, office systems, and ways of working.
