Work as part of a multi-disciplinary team in a first-contact patient facing coaching roleto provide personalised support to individuals, their families, and carers to empower them to manage their own wellbeing and live independently. Criteria includes supporting individuals experiencing challenges affecting wellbeing (e.g. anxiety, stress, low mood, life events). The role does not provide clinical treatment, diagnosis, or therapy.
Engage patients in coaching, motivating them through multiple sessions to identify their needs, set goals, and achieve their personalised wellbeing objectives. Provide non-clinical interventions, including self-management education and peer support. Support personal choice and encourage steps to improve wellbeing, without making clinical risk decisions. Work in partnership with social prescribing services to connect patients to community-based activities or support charities, enabling them to take greater control of their wellbeing.
Manage and prioritise a caseload of patients, taking a non-judgemental, person-centred approach that emphasises communication. Refer patients to other health professionals within the PCN when needs exceed coaching scope (e.g., GP, social prescriber, or mental health practitioner). Referrals should not include private services. The coach does not provide therapy or manage clinical cases.
