The post holder is a Mental Health & Wellbeing Coach, who works alongside the general practice team and a mental health multi-disciplinary team, providing non-clinical coaching and wellbeing support only within NN4 PCN.
The individual will act within their professional boundaries to work for the benefit of patients, providing coaching support to improve wellbeing without providing clinical treatment.
They will provide assistance and advice to other members of the primary healthcare team in managing the mental wellbeing of the practice population.

Special Requirements of the Post:
Participate in regular individual and group coaching-focussed supervision with a suitably qualified supervisor.
Develop collaborative relationships and work in partnership with health, social care, community, and voluntary sector providers, as well as multidisciplinary teams, to support patients wider health and wellbeing and contribute to reducing health inequalities.
Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care and behavioural approaches, and ensuring consistency in the follow-up of patients goals with MDT input.
Raise awareness within the PCN of shared-decision making and decision support tools.
Demonstrate understanding of safeguarding processes for vulnerable people.
Implement basic safeguarding processes for vulnerable individuals.
Job description
Work as part of a multi-disciplinary team in
a first-contact
patient facing coaching role
to provide personalised support to individuals, their families, and carers to empower them to manage their own wellbeing and live independently.
Criteria includes supporting individuals experiencing challenges affecting wellbeing (e.g. anxiety, stress, low mood, life events). The role does not provide clinical treatment, diagnosis, or therapy.
Engage patients in coaching, motivating them through multiple sessions to identify their needs, set goals, and achieve their personalised wellbeing objectives.
Provide non-clinical interventions, including self-management education and peer support.
Support personal choice and encourage steps to improve wellbeing, without making clinical risk decisions.
Work in partnership with social prescribing services to connect patients to community-based activities or support charities, enabling them to take greater control of their wellbeing.
Manage and prioritise a caseload of patients, taking a non-judgemental, person-centred approach that emphasises communication.
Refer patients to other health professionals within the PCN when needs exceed coaching scope (e.g., GP, social prescriber, or mental health practitioner). Referrals should not include private services. The coach does not provide therapy or manage clinical cases.

Experience
Essential
Minimum of 2 years experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field.
An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing.
Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
An appreciation of the new NHS landscape including the relationship between individual providers, PCNs and commissioners.
Desirable
Experience of supporting individuals with mental health and wellbeing needs.
Understanding of evidence-based wellbeing and behaviour change approaches.
Understanding of the mentorship process.
