We have an exciting opportunity for a Primary Care Nurse to join our Primary Care Team at HMP Lancaster Farms, a category C Prison located in outskirts of Lancaster, Lancashire.
HMP Lancaster Farm consists of 550 males.

Hours:

• Monday - Sunday service - 37.5 hours per week
• Monday -Thursday 7.30am -7.30pm
• Fri 7.30am - 6.00pm
• Weekends and BH 8.00am - 5.30p
• One weekend every 6-8 weeks

Salary:
This position is full time, you'll receive an annual salary up to 34,428 - 41,659, per annum FTE - depending on experience.

What you'll be doing
As a Primary Care Nurse your responsibilities will vary. You will

• Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
• Deliver non-judgemental care that makes a real difference
• Inspire excellence by forging strong links with our partners and stakeholders
• Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
• Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Qualifications required:

• Registered nurse - Adult - NMC

• Evidence of Continued Professional Development
• Understanding of the complexities of multi-disciplinary and multi-agency working

• Excellent communication skills
