Job Description

We have an exciting opportunity for a Primary Care Nurse to join our Primary Care team at HMP Drake Hall, a women's closed prison, located near the town of Eccleshall in Staffordshire with around 340 females inmates.
Hours

• Monday - Friday 7.15am - 5.15pm
• Weekends 7.30am - 5.30am
• Every Other weekend
• 4 week rolling rota

Salary
This position is full time, you'll receive an annual salary between 34428 - 41659 per annum FTE - depending on experience.
Some duties include:

• Support with Health Screening and Monitoring

Supporting Health Education and Promotion

• Act as a Consistent, Trustworthy Point of Contact
• Monitor Ongoing Health Needs
• Liaise with Multidisciplinary Teams in primary and secondary care with a clinical understanding of need and prioritisation.
• Support Transition and Release Planning

What you'll be doing
As a Primary Care Nurse your responsibilities will vary. You will

• Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary teams
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
• Deliver non-judgemental care that makes a real difference
• Inspire excellence by forging strong links with our partners and stakeholders
• Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
• Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Qualifications required:

• Registered nurse - Adult - NMC

• Evidence of Continued Professional Development
• Understanding of the complexities of multi-disciplinary and multi-agency working

• Excellent communication skills
