We have an exciting opportunity for a Primary Care Nurse to join our Primary Care team at HMP Oakwood, a category C Prison located in Wolverhampton.
HMP Oakwood is one of the largest prison in the UK, housing over 1,500 men.

Hours:

• This role is 37.5 hours per week:
• Monday - Sunday service
• 4 week rolling rota

Monthly rota:

• WK1 - Mon & Tue - 7am - 6pm, Sat - 7.30am - 8pm, Sun - 7.30am - 5.30pm
• WK2 - Weds & Thurs 7am - 6pm, Fri 7am - 7pm
• WK3- Mon & Tue - 7am - 6pm, Sat - 7.30am - 5.30pm, Sun 7.30am - 8pm
• WK4 - Weds & Thurs 7am - 8pm, Fri - 7am - 6pm

Salary
This position is full time, you'll receive an annual salary between 34,428 - 41,659 per annum FTE - depending on experience.

What you'll be doing
As a Primary Care Nurse your responsibilities will vary. You will

• Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
• Deliver non-judgemental care that makes a real difference
• Inspire excellence by forging strong links with our partners and stakeholders
• Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
• Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Qualifications required:

• Registered nurse - Adult - NMC

• Evidence of Continued Professional Development
• Understanding of the complexities of multi-disciplinary and multi-agency working

• Excellent communication skills
