Trent Meadows is seeking an experienced Primary Care Pharmacist to become a valued member of our dynamic Pharmacy team.

About You:


The ideal candidate will have completed the CPPE course, hold prescriber status, and possess a minimum of three years' post-qualification experience. You will be confident managing long-term conditions, working directly with patients to assess and treat ailments, and encouraging self-care. A solid understanding of Structured Medication Reviews (SMRs) and the strategic delivery of primary care targets is essential.

Key Skills:


You will demonstrate excellent verbal and written communication, have experience influencing clinical decisions, and be skilled in conducting medication reviews.

Role Overview:


As part of a multidisciplinary team, you will help develop and manage key processes such as repeat prescription reauthorisation, medicines management during care transitions, and safer prescribing systems. You will collaborate closely with GPs, Advanced Nurse Practitioners (ANPs), and nurse prescribers who will provide support.

What We're Looking For:
