Project Worker - Community Living

Job reference: AHSS00264D1AGE Location: Community Living, Rhyl Salary: Grade 5, 14.35 - 15.56 per hour (subject to validation) Hours: 28 per week Contract: Permanent
This is an exciting opportunity to join our dedicated team of staff in supporting individuals with complex disabilities who live in their own home or shared accommodation in the community. To support each individual to meet their goals and outcomes using a person centred approach.

We are looking for energetic and motivated staff who want a challenge. Staff will need to adapt and learn what the support works for the individual citizens, ensuring choice and control is at the forefront of all support delivered.

Appointment subject to Disclosure & Barring Service Check, satisfactory references and registration with Social Care Wales.
