Job Description

The Psychiatric Liaison Service based at Epsom Hospital is seeking an enthusiastic, highly motivated older adults specialist nurse to join our team. This is a Band 6 position.



We are a multi-disciplinary and diverse team offering assessment and signposting service. We are a 24/7 service and we work with all ages 18 and above.



We work very closely with acute hospital staff, local MH services, etc. and offer regular education and training programmes to colleagues.



Our service is PLAN (Psychiatric Liaison Accreditation Network) accredited.



We value all staff, we offer weekly teaching in our team for professional development and offer opportunities to attend courses for personal and professional development. Our staff will receive regular supervision as well as appraisal. You will be expected to complete the practice assessor and supervisor training courses and offer supervision to student nurses and other learners in the team.



This role is suitable for a practitioner with extensive experience in older adults who is passionate about ensuring a high quality holistic approach to the care and support provided to people with mental health difficulties in the general hospital.

To perform Bio- psychosocial assessments of people referred to the Liaison service from a variety of sources within Epsom General Hospital, including A&E and Medical Wards.

To work with people presenting with a range of psychosocial needs arising in the context of physical illness, as well as people presenting to the hospital following self-harm or in a mental health crisis

To provide rapid psychiatric assessment and safe discharge planning in Emergency Department. To agree on a collaborative plan of care with people who use our services and their carers, where appropriate and arrange appropriate after-care To ensure that the relatives, carers and friends are actively encouraged to participate in care, provided with information, help and support where necessary To mentor, supervise and teach qualified and unqualified staff including students To lead the care for a specified group of service users such as frequent A&E attendees To assist in the development and maintenance of good working relationships with general hospital staff, local mental health services, primary care and non - statutory organisations To facilitate the teaching and training of mental health topics in the general hospital.
