? Key Responsibilities
In this role, you will:

• Conduct comprehensive assessments of adults' care and support needs
• Coordinate safe and timely hospital discharges in partnership with NHS colleagues
• Manage a varied caseload with autonomy and professionalism
• Develop individualised care plans that promote independence, dignity, and empowerment
• Work in line with current legislation and best practice in adult social care
• Build trusted relationships with service users, carers, and multi-agency teams
• Offer support and guidance to unqualified staff and students, helping to build capability across the team

? Who We're Looking For
We're looking for someone who is:

• A Qualified Social Worker registered with Social Work England
• Experienced in adult social care, particularly in hospital discharge or community settings
• Skilled in risk assessment, care planning, and multi-disciplinary collaboration
• Knowledgeable in relevant legislation and frameworks
• A clear communicator, problem solver, and advocate for vulnerable adults
• Proactive, compassionate, and committed to achieving positive, sustainable outcomes

Minimum: 4+ years post-qualification experience preferred.
