Qualified Social Worker, Palliative Care Team
Pay rate to 28.22 per hour
Contract role
We are recruiting for an experienced Social Worker to work in a team across Calderdale and Kirklees.

  • The palliative care social work team, work collaboratively with Health to support adults (18+) and their family and carers who reside across Calderdale and Kirklees (if health funded with a Calderdale GP).
  • Social workers within this service provide a vital role in ensuring that statutory duties are met under the Care Act, to provide support to people who meet the Care Act national criteria for adults requiring support due to an end of the life prognosis.
  • The role will primarily work with individuals and their families / carers who require support with discharge planning from hospital where NHS fast track funding has been agreed, due to a rapidly deteriorating health condition. The team will work closely with the hospital discharge team and the Integrated care board to complete appropriate assessments and coordinate services for timely and safe discharges.
  • The team also work with individuals and their families / carers in the community and local hospice, where they have a palliative prognosis of less then 12 months. Tasks include Care Act Assessments in line with Mental Capacity and Human Rights legislation, risk assessments, continuing health care checklists and Decision Support Tool meetings. Safeguarding enquiries under s42 Care Act.
Description of Role
Develop and lead forward investment programmes to meet EPC C by 2030 and Net Zero Carbon by 2050 for council homes.
Act as the Council's lead professional on environmental sustainability, retrofit, funding, and decarbonisation for Housing.
Secure, manage and maximise external investment, including SHDF, ECO, Warmer Homes, and consortium bids.
Develop the borough's housing-related energy, retrofit and sustainability strategies, guiding senior leaders through evidence-based investment decisions.
Scope, design and deliver sustainability, retrofit and low-carbon programmes, ensuring legislative compliance and sector best practice.
Influence, collaborate and build partnerships locally, regionally and nationally on sustainability, funding and retrofit.
