We have an exciting opportunity to join the Growing Space team to be part of an innovative role.

The Rainbow Connector role will support the mental health and wellbeing of individuals referred into the service by using guidance from the "five steps to wellbeing". During 3 home visits, the RC will provide a number of bespoke resources that will help people in our communities who may be facing issues such as social isolation.

We are looking for a caring, flexible, self-motivated individual who has experience of working in the care sector, who would enjoy being part of an innovative program that makes a real difference to the people who access this service.

1. Carry out 3 in-person contact visits offering support that promotes social connectedness.

2. To use the Five Steps to Wellbeing initiative as a framework for human contact and emotional support.

3. Be able to sign post to relevant organisations or services as deemed appropriate to the needs of the individual.

4. To liaise closely with members of partner agencies and other external organisations in order to promote the RC service, and support individuals that may benefit from additional services.

5. To source suitable stock, and manage quantities to create Activity packs to support adults in the community and recovery wards, requiring support through the Rainbow Connector initiative.

6. Report to and work with Senior Rainbow Connector closely to monitor referrals and progress of the project.

7. To work to and deliver targets as required, ensuring that all records are kept up to date on the organisations database. Information about projects, staff and clients is kept confidential in accordance with GDPR guidelines.

8. To ensure that all procedures relating to the project are dealt with in accordance with Growing Space policies and procedures.

9. To maintain personal and continual professional development to meet the changing demands of the role and participate in appropriate training/management meetings.

10.To take reasonable care for the health and safety of yourself and other persons who may be affected by your acts and omissions, and to comply with health and safety legislation as appropriate.

11.To work in the context of equality and diversity and pursue inclusive policies and practices, actively supporting the principles and practice of the equality opportunities act.

12.To undertake any additional duties as deemed appropriate by the line manager.

13.Full clean driving licence and must have access to a vehicle.

Post subject to Enhanced DBS Check
