About the Role
Are you passionate about enabling adults to live independently, safely, and well in their own homes? Southampton City Council's Reablement Service is seeking a dedicated Reablement Assessor to join our team on a 6-month fixed term basis.
As a Reablement Assessor, you will:

  • Work with individuals and their families/circles of support, having conversations about what matters most to them.
  • Complete strengths-based assessments that promote independence, health, and wellbeing.
  • Create and review outcome-focused plans, negotiating and coordinating resources to meet needs.
  • Prevent harm and manage risk in line with the principles of Making Safeguarding Personal, keeping the person at the centre of planning and promoting positive risk-taking.
  • Help people build resilience and achieve outcomes important to them by providing accurate support, information, advice, and guidance, and connecting them to wider networks of opportunity and support.
  • Collaborate with colleagues and partner agencies to ensure a high-quality, responsive service leading to positive outcomes for adults with care and support needs.
About You
We're looking for someone who:
  • Holds a Level 3 Certificate in Health and Social Care (essential).
  • Has experience working with adults in a social care setting, ideally with knowledge of strengths-based approaches and outcome-focused planning.
  • Demonstrates excellent communication skills and the ability to build positive relationships with service users, families, and colleagues.
  • Is confident in managing risk and safeguarding, with a commitment to promoting independence and wellbeing.
  • Is proactive, resilient, and able to work both independently and collaboratively.
  • Is committed to continuous professional development and reflective practice.
  • Is able to work a 2-week rota pattern, including weekends and unsocial hours as required.
  • Is a driver with access to a vehicle for work purposes (essential).

Salary: The salary band for this role is 37,280 - 40,778. The starting salary is 37,280 with annual progression through the salary bands.
