4 Outcomes
The post holder will be expected to:
Assess competence and the embedding of a Reablement ethos and best practice, across the fields of enablement rehabilitation and vocational rehabilitation
Assessment and provision of minor adaptations which promote independent living
Promotion of the role of Occupational Therapy, Physiotherapy and Nursing in the specific areas or enablement, rehabilitation and vocational rehabilitation
Promotion of a culture and ethos of enablement and rehabilitation by constructively challenging existing behaviours, attitudes and practices
Prioritising and allocation of referrals in relation to rehabilitation and enablement to ensure the most appropriate use of service resources
Assess, plan, monitor and evaluate enablement and rehabilitation service provision on an individual and team basis to ensure positive outcomes for service users
Empowering service users (including staff) to adopt a pro-active and preventative approach to independent and functional living
Lead the promotion and development of services to maximise meaningful activities and functioning of service users
Liaising and working in partnership with a range of agencies and stakeholders
Provide and develop services for the specific needs of service users including staff
Ensure continued professional development and maintain a professional registration
5 Tasks and Responsibilities
The post holder will be expected to undertake a range of tasks as appropriate to the role and responsibilities. These may include all or some of the following but are not restricted to:
Rehabilitation & Enablement Specific
Deliver an experiential enablement programme across a range of BAC settings
Supporting the workforce to develop knowledge, skills, attitudes and behaviours consistent with an enablement and rehabilitation approach
Promoting recovery and the overcoming of barriers to participation in daily activities, independent living
Educating and promoting best practice in recording, documenting and record keeping of service user notes and support plans
Assessment and provision of specialist equipment and minor adaptations to promote independent living.
Vocational Rehabilitation Specific:
Educating and promoting a preventative and self-management approach to staff in order to maintain health and well-being
Assessing the functional, psychological and emotional needs of staff and how this impacts on their work
Promoting recovery and overcoming of barriers to participation in work through assessment and intervention
Service Provision
Undertake clinical facilitation sessions with a range of appropriate staff to ensure best practices are followed in the areas of rehabilitation and enablement
Assessment of service user needs
Effective planning, implementation, monitoring and evaluation of plans to ensure service user needs are met
Reviewing referrals and prioritising referrals and admissions effectively where appropriate
Provide leadership and day to day management, advice, support and guidance to team members in order to meet the needs of the service
Health and Safety
Responsibility for monitoring and ensuring health and safety of staff, service users, buildings/community
Responsibility for the processing and notification of Accident and Incident Reporting
Forms where applicable
Participation in ASP procedures and follow up on ASP Reporting Forms where applicable
Ensure that all health and safety regulations are adhered to, including carrying out risk assessments and regular auditing so that a safe and secure environment is provided and maintained

Staffing
Providing guidance and support to staff from a range of services
Monitoring the quality and impact of interventions
Quality Assurance and Service Improvement
Take a lead in quality assurance processes including service user engagement and outcomes
Development and improvement of the service
Take a lead in implementing audit action plans
Ensure compliance with requirements of regulatory bodies
Respond proactively and timeously to concerns or complaints
Monitor quality and consistency of service user risk assessments, plans and outcomes
Ensure that services are delivered in accordance with relevant legislation, policies and procedures and national care standards and professional codes of practice

Report statistical information on service delivery as required

9 Requirements of the Job
The post holder needs to hold as a minimum:
Professional qualification in Occupational Therapy, Nursing or Physiotherapy
Be HCPC registered
PDA in supervision or willingness to work towards as role required
Registration with relevant professional body
PVG registration
The post holder may be expected to:
Work flexibly to meet the demands of service users and the service
Work at alternative locations as required
