An exciting opportunity has arisen to join Intermediate Care Service in Walsall, this is a multi-disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care.
We are particularly looking for enthusiastic and committed individuals to join our team to help shape and deliver our services, and contribute to the model of best practice. The successful candidates will be required to have experience of working in Adult Social Care and to join our Multi-Disciplinary Team.
You will be required to support customers to develop or regain skills and confidence, to co-ordinate and deliver a programme of active Reablement to support customers to achieve their full potential, working in accordance with individualised Reablement Plans. To work cohesively within a multidisciplinary team to maximise customer independence and reduce the need for ongoing support. To support customers to enjoy full access to the resources in their community, develop informal support networks.
Minimum Requirement Qualification for this post NVQ L3 Health and Social Care Or equivalent qualification.
In return, we shall offer you access to a supportive Team Manager and Area Managers, regular supervision and good peer support. You will have an opportunity to enhance and develop your skills, as Walsall offers a range of training and development opportunities.
