Job Overview:

We are currently seeking a motivated and experienced Reablement Lead to join our growing team. The Reablement Lead will be responsible for coordinating and overseeing the delivery of reablement services within the Suffolk County Council Reablement contract, ensuring outcomes are achieved in line with client goals, service specifications, and Key Performance Indicators (KPIs). This role requires excellent assessment skills, a strong understanding of short-term reablement pathways, and the ability to motivate and lead a team to deliver exceptional, outcome-driven care.

As a Reablement Lead, you will play a key role in managing and supporting the delivery of the reablement service -- ensuring each client receives tailored, short-term support focused on promoting recovery, independence, and confidence. You will conduct comprehensive assessments, develop individualised goal-based support plans, and monitor progress in collaboration with clients, staff, and multidisciplinary professionals.

Key Responsibilities:

Conduct comprehensive reablement assessments for new referrals, focusing on clients' strengths, abilities, and personal goals. Develop, implement, and review tailored reablement plans in collaboration with clients, families, and professionals. Work in partnership with Suffolk County Council, community health teams, and other agencies to achieve positive outcomes. Monitor and evaluate client progress throughout the reablement period, ensuring goals are realistic, measurable, and person-centred. Supervise and support reablement staff, ensuring high standards of care delivery and adherence to company policies and CQC requirements. Maintain accurate and detailed records of assessments, progress reviews, and outcomes in line with SCC and CQC standards. Ensure timely completion of documentation required for KPI and contractual reporting. Promote a positive, enabling culture that encourages clients to regain confidence and independence. Support the Registered Manager and Senior Team in achieving service objectives and continuous improvement initiatives.
Requirements:

Previous experience in a reablement, enablement, or rehabilitation role within health or social care. A relevant qualification in Health and Social Care (minimum NVQ Level 3 or equivalent). Excellent assessment and goal-setting skills. Strong understanding of outcome-based practice and short-term reablement models. Ability to lead, motivate, and support staff effectively. Excellent communication, interpersonal, and organisational skills. Ability to work under pressure, manage caseloads, and adapt to changing service demands. A full UK driving licence and access to a reliable vehicle. A compassionate, person-centred approach with a genuine commitment to supporting people to live independently.
