About the Role
We have a new opportunity to work as a Reablement Response Care Worker.
For this role you must have care experience and be a driver with a full, clean driving license and access to own vehicle.
As a Reablement Response Care Worker, you will help individuals regain independence and confidence after experiencing a decline in their health following a hospital discharge.
You will be covering Chester and the surrounding areas i.e. Ellesmere Port, Malpas, Blacon, etc.
You will be responsible for providing short term care to help get customers back on their feet on their return home from Hospital. You will helps customers to rebuild their confidence in day-to-day tasks and allows them to return to a normal independent life.
12.40 - 12.70 per hour- Block Pay.
You will also receive a drivers enhancement of 50p per hour on top of your pay rate.

Job Opportunity
You will need to be an experienced care/support worker who has the drive to work with individuals to develop tailored support plans that address their specific needs and goals.
