Referral Co Ordinator / Clinical Coder

Job Description

Primary key responsibilities The following are the core responsibilities of the clinical coder. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels. Typing letters, reports and associated documentation as required Liaising with external agencies such as hospitals and community services, ensuring referrals are processed efficiently Initiate contact with and respond to requests from patients, team members and external agencies Maintaining an accurate referrals database Processing referrals using the electronic referral system (ERS) Processing requests for information i.e., SAR, insurance/solicitors letters and DVLA form Input data into the patients healthcare records as necessary, Entering SNOMED CT code data where necessary Accurately clinically coding data on EMIS web into the patient record Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action Inputting and extracting data as required to support practice report Filing and storing records as require Photocopying documentation as required Action all incoming correspondence, emails and mail in a timely manner and directing to the appropriate person Scanning patient related documentation and attaching scanned documents to patients healthcare records Summarising new patient records Answering incoming telephone calls, transferring calls or dealing with the callers request appropriately Managing all administrative queries as necessary Answering incoming professional line phone calls, transferring calls or dealing with the callers request appropriately Maintaining a clean, tidy, effective working area at all times Secondary responsibilities In addition to the primary responsibilities, the clinical coder may be requested to: Review and update the practice coding and referrals policies as require Partake in audit as directed by the audit lead Carrying out system searches as requested Undertake all mandatory training and induction programmes Attend a formal appraisal with line manager at least every 12 months.



Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed Contribute to public health campaigns (e.g., flu clinics) through advice or direct care
