Purpose of Role
To design, coordinate, and deliver personalised, meaningful, and engaging activity programmes for adults and children with complex needs, mental health conditions, and learning disabilities within their homes and supported living communities. This role fosters wellbeing, independence, and inclusion through the creation of bespoke group and individual activity plans.
Key Responsibilities
Plan, develop, and lead creative and tailored activity programmes, including community outings, events, holidays, and in-house sessions, for diverse client groups (adults, children, and young people with complex/mental health needs).
Conduct regular person-centred assessments with clients and families to understand interests, preferences, abilities, and any risk considerations.
Produce individual and group activity plans, ensuring each is appropriate, stimulating, and supports personal goals, independence, and social interaction.?
Arrange and coordinate large-scale events, themed days, and accessible group trips, liaising with external venues, entertainers, and volunteers.
Work independently to schedule, organise, and deliver activities across multiple homes and supported living settings, managing your own diary and logistics.?
Foster an inclusive, empowering, and empathetic atmosphere--promoting engagement for people with varied communication needs and backgrounds.?
Maintain accurate records of participation, risk assessments, consent, and feedback; adapt activities based on reviews and outcome evaluations.
Collaborate with care/support teams, therapists, and external partners to enhance and individualise the activities offer.
Ensure all activities adhere to relevant safeguarding, health and safety, and organisational policies.
Advocate for service users and actively engage with community resources and local events to extend opportunities for participation.
Support company culture by contributing to regional events, campaigns, and promotion of best practice in activity provision.
Person Specification
Proven experience coordinating activities or events for vulnerable adults and/or children, ideally within health, social care, or community support settings.?
Passionate, energetic, and creative, with the ability to motivate and engage individuals and groups.
Demonstrable understanding of person-centred approaches and creating bespoke support/activity plans (including for mental health and complex needs).
Strong organisational and diary management skills; able to work autonomously and flexibly across regional sites.
Excellent communication, partnership, and problem-solving skills.
Empathetic, approachable, and professional manner at all times.
Full UK driving licence and willingness to travel regionally (mileage reimbursed).
