Purpose of the Role
To provide high-quality, person-centred nursing care to residents, supporting their dignity, independence, and overall wellbeing. You will deliver compassionate, evidence-based care in line with clinical guidelines, professional standards, and our organisational values.

Main Responsibilities1. Clinical Assessment & Care Planning
Complete comprehensive clinical and risk assessments.
Develop and review individualised, person-centred care plans with resident and/or family input.
Ensure 'Resident of the Day' responsibilities are completed to standard.
Administer medications and clinical interventions (e.g. dressings, enteral feeds, ECGs, venepuncture, cannulation) according to protocols.
Respond to clinical changes, emergencies, and plan discharges or external referrals as needed.
Delegate appropriate care tasks to trained team members.
2. Multidisciplinary Teamwork & Collaboration
Work closely with GPs, allied health professionals, hospitals, and social care teams.
Mentor and support junior staff and care assistants.
Lead handovers with accurate, up-to-date documentation.
Ensure information is effectively shared across shifts and departments.
Act as the Named Nurse for assigned residents, coordinating care with their key workers.
Support care staff during busy periods to ensure quality of care is maintained.
Represent the home professionally in meetings with families and professionals.
Act as a champion in key care areas (e.g. dementia, oral care, falls prevention).
3. Clinical Governance & Safety
Maintain accurate, timely clinical records in line with GDPR and NMC guidance.
Adhere to policies on infection prevention, health & safety, and safeguarding.
Contribute to audits, investigations, and quality improvement initiatives.
Escalate concerns appropriately and participate in reflective practice.
4. Professional Development & Leadership
Maintain up-to-date NMC registration and meet revalidation requirements.
Engage in CPD and additional training to enhance clinical practice.
Lead by example and promote a learning culture among colleagues.
Contribute to service development and review of policies and procedures.
Support with induction of new staff and ensure lessons learned are shared constructively.
Use digital systems effectively for care planning, handovers, and documentation.
5. Health Monitoring & Incident Reporting
Monitor and respond to changes in residents' physical or mental health.
Respond promptly to emergencies and call bells.
Complete and follow up on incident and accident reports within digital care planning systems.
Keep care plans, risk assessments, and handover notes up-to-date in real time.
Ensure digital handover notes are clear and accessible to the full care team throughout the shift.
Person Specification
Qualifications & Registration
Registered Nurse (RGN/RMN/RNLD) with valid NMC registration
UK-recognised nursing qualification (Degree/Diploma)
Additional post-registration training (desirable)
Skills & Experience
Previous experience in a care home or similar setting
Strong clinical skills with the ability to assess, plan, and evaluate care
Excellent communication, leadership, and teamwork abilities
Working knowledge of safeguarding, the Mental Capacity Act, and CQC regulations
Proficient in using digital care systems and electronic documentation
