at our The Avenue Specialist Nursing Care Home.

About the service:

Located in the vibrant heart of Northampton, The Avenue is a welcoming nursing care home in Northampton, that offers homely and comfortable care for older people experiencing all forms of Dementia and elderly residents with clinical needs.

Our service supports a wide range of care pathways, including:

Continuing care for residents with longstanding, chronic mental and physical health needs Dementia care including early onset dementia care Parkinson's disease Huntington's disease care Complex care needs with behaviours that challenge

Key Responsibilities of the Role:

Coordinating the planning and implementation of patient care Developing and coordinating individual or group therapeutic activities Managing aspects of medication management in line with the Trust and NMC guidelines Participating in clinical review meetings Promoting, fostering, and ensuring good working relationships Adhering to all NMC, CQC and local authority standards and guidelines including the NMC code of professional conduct Acting as an advocate for our patients and residents
