We are seeking a dedicated Registered Mental Health Nurse to join our innovative Mendip Assertive Outreach Team (AOT). In this role, you will conduct comprehensive assessments and deliver psychosocial, evidence-based interventions to individuals experiencing severe and enduring mental health challenges, supporting them on their path to recovery.


You will be part of a dynamic service that has redefined the traditional boundaries between primary and secondary care. Working closely with service users, carers, the voluntary sector, and community enterprises, you will help deliver integrated, person-centred care that makes a real difference.

Main duties, tasks & skills required

Deliver a high-quality, comprehensive mental health service to individuals with complex and enduring needs within the community. Manage a defined caseload, carrying out assessment, planning, implementation, and evaluation of care in collaboration with service users and carers. Provide psychosocial interventions and support to help service users manage daily living, finances, housing, and community engagement. Offer clinical advice, leadership, and supervision to junior staff and students, supporting their professional development. Work collaboratively with inpatient units, CMHTs, HTTs, primary care, and partner organisations to ensure continuity of care. Promote service user involvement in care planning, reviews, and service evaluation. Participate in service development, audit, and quality improvement initiatives.

Job description

As a Band 6 Nurse within the Assertive Outreach Team, you will:

Provide specialist care:

Undertake comprehensive mental health assessments and create person-centred care plans that are measurable, outcome-focused, and regularly reviewed.

Deliver evidence-based interventions:

Support service users through therapeutic engagement, problem-solving, and psychosocial interventions, ensuring care is safe, effective, and recovery-focused.

Support families and carers:

Work collaboratively with families and significant others, recognising their role in recovery and providing appropriate guidance and support.

Exercise clinical leadership:

Supervise and mentor junior staff, contribute to preceptorship and student placements, and support team development in partnership with the Team Manager.

Promote integrated working:

Build strong links with inpatient services, CMHTs, GPs, and voluntary/community organisations to provide joined-up, accessible care pathways.

Ensure safe and professional practice:

Adhere to Trust policies, the NMC Code of Conduct, and relevant legislation including the Mental Health Act. Maintain accurate and timely electronic records of all care delivered.

Contribute to service improvement:

Engage in audit, evaluation, and research activities to help develop the Assertive Outreach model and ensure the team continues to deliver outstanding care.

Professional growth:

Access regular clinical and managerial supervision, training, and CPD opportunities to develop your skills in assertive outreach and community mental health nursing.

Essential




RMN - current registration with relevant professional body


Commitment to Continuing Professional Development

Desirable




Evidence of recent relevant training.


Mentor qualification

Post qualification experience in mental health


Experience at Band 6 level or experience in community mental health teams at band 5


Experience of care planning

Good communication skills (both written and verbal) in dealing with people at all levels


Ability to supervise staff.

Ability to lead a team, demonstrating teaching and mentorship skills and work effectively as part of a team.
