Job Description

We are currently looking for a positive, proactive, experienced RMN to join our Team at the Health Based Place of Safety (HBPoS) - Mausdley Hospital.



The HBPoS service is the single point of admission in the trust for all persons requiring care in a hospital-based place of safety. It operates from a specially designed unit situated at the Maudsley Hospital on a 24/7 basis and staffed by a dedicated multidisciplinary team.



The multi-disciplinary team is dynamic and diverse, including RMNs, CSWs, an OT, doctors and experienced consultants who work closely to ensure better patients outcomes.



Working as part of the clinical team the post holder will make appropriate use of the latest evidence-based practice to deliver person centered care within the team and across all inpatient wards. The post holder with be part of the Quality Improvement ethos to work with the team to continually improve the service to enable the best possible patient outcomes.



The post holder will deliver high quality safe nursing care, ensuring

consistently high standards of practice and clinical excellence which comply with the Nursing and Midwifery Council (NMC) code of professional conduct.



You will provide care for presenting service users from initial contact and assessment, formulating and delivering care plans, and evaluation of care. You will assess the risks and needs of service users to plan and evaluate care using a collaborative approach with service users and their carers. Providing accurate information about care in an accessible format to patients and their relatives



The post holder will co-ordinate a first-class service that includes assessment and monitoring of physical health, maintaining service users' rights, supervising junior staff and students, working collaboratively within multi-disciplinary teams and maintaining a safe and therapeutic working environment and accurate records.



South London and Maudsley NHS Foundation Trust (SLaM) provide the widest range of NHS mental health services in the UK as well as substance misuse services for people who are addicted to drugs and alcohol. We work closely with the Institute of Psychiatry, Psychology and Neuroscience (IoPPN), King's College London and are part of King's Health Partners Academic Health Sciences Centre.



SLaM employ around 5000 staff and serve a local population of 1.1 million people. We have more than 230 services including inpatient wards, outpatient and community services. Currently, provide inpatient care for approximately 5,300 people each year and treat more than 45,000 patients in the community in Croydon, Lambeth, Lewisham and Southwark; as well as substance misuse services for residents of Bexley, Bromley and Greenwich.

Developing the management of care needs through:

Effectively screening referrals and initial data collection.



Monitoring of individual service user / patient progress



Feedback and discussion within the team and collaborating with other teams and individuals to promote learning and personal development.



Devising a plan of care and interventions supervised by a Clinical Charge Nurse



Leading on and directing junior to staff in providing hands-on care to service users including:



Undertaking 1-1 engagement with service users in an honest, open and non- judgmental



Recognising and valuing service users as individuals, acknowledging the importance of maintaining the service user's respect and dignity at all times and ensure that the care provided respects equality and Developing care plans that reflect service user needs and relevant patient care documentation working in partnership with service users, wider MDT and friends/relatives/significant.



Ensuring the monitoring and recording service user clinical observations are undertaken, such as Physical observations and vital signs and treating or escalating as appropriate any signs or symptoms of deterioration or that do not fulfill the personal parameters to the relevant professional for treatment. Promote healthy lifestyles and give health promotion advice and Implementing the principles of 'recovery': encouraging and supporting service users to engage with healthy lifestyles, facilitating psychosocial activities and other activities of their interest. Where appropriate contributing to psychological



Facilitating the admission and discharge of service users in line with the Pan London section 136 pathway, ensuring needs of the patient and service including relevant legislative or required documentation are:



Advocating for service users' needs and rights within Trust Encouraging and empowering service users to have an optimum level of responsibility for their individual programme of care.



Regularly liaising with other professionals such as Police, Acute Referral Centre, Psychiatric Liaison services and other mental health organisations to ensure effective patients' assessments are carried out with the aim of achieving a safe and positive outcome.
