Registered Mental Health Nurse (RMN) and/or Learning Disabilities Nurse (RNLD). Band 5, 6 & 7. Reports to: Clinical Services Manager Location: Greater Manchester - with willingness to travel to homes. Hours: 9am-5pm Monday-Friday 37.5 hours (Will consider flexible working arrangements for healthy work/life balance).

Mental health Case management and Care Coordination JOB DESCRIPTION: Nestlings Care LTD are looking for Registered Mental Health Nurses (RMN) and/or Learning Disabilities Nurses (RNLD) with CAMHS experience to join our highly skilled multidisciplinary team. As a qualified nurse, your focus will be to provide the highest standard of person-centred care, promoting the best quality of life for all of our service users. Nestlings Care Ltd is an independent organisation registered with CQC and Ofsted to provide specialist residential placements for young people and young adults with mental health difficulties. We offer intensive, Individualised community care with 24 hour mental health crisis management.



We have identified that there are a number of young people and young adults with complex mental health difficulties who are either deemed unsuitable for inpatient hospital settings, have recurrent hospital admissions or are delayed discharges from hospital due to inadequate or delayed access to mental health services in the community. We offer specialist therapeutic placements with an in-house multi-disciplinary mental health team. We provide holistic care within the community in order to empower young people to be independent and enjoy a good quality of life. Qualifications, Experience and Skills: First level registration ability to demonstrate knowledge and skills applicable to the post as per job specifications.



Ability to demonstrate good interpersonal communication skills to promote and maintain relationships with young people, carers, cares teams at placements and stakeholders. Develop transition, admission and discharge plans , follow up referrals and assessment for suitability for the services. Ability to offer comprehensive mental health assessments underpinned by evidenced based knowledge and collates information with a view to have comprehensive case summary. Ability to formulate care plans, risk management plans and formulate psychosocial treatment plan.



Out of hours on call for crisis management, on a rota system (1:4). Facilitate treatment planning in liaison with external agencies. Referral and liaison with secondary care. Knowledge of MHAct, Mental Capacity Act and Deprivation of liberty safeguards.



Ensure timely and accurate documentation and maintain confidentiality within legal, organisational and ethical requirements. Ability to manage high risk and complex presentation in community with positive therapeutic risk management. Ability to make therapeutic relationship with young people. Ability to undertake medication management, medication audits and offers medication competencies to care staff.



Understanding of Mental Health Act and monitoring of required paperwork. Ability to work with wider multiagency professionals and commissioners. Actively utilising strategies to enhance Recovery for young people and young adults. Participates in continuous quality service improvement initiatives and demonstrates flexibility to adapt to service changes to monitor and improve standards Ability to undertake care planning, implementation and evaluation of care offered.



Inpatient experience desirable. All newly qualified (band 5 nurses) will be considered for this role and provided with extensive supervision from a band 7 RMN. Communication: Working as part of specialist mental health team at Nestlings Care LTD. Working in partnership with specialist residential staff for cohesive care and treatment.



Required to have effective and accurate written and verbal communication skills. Ensure that all communication is in line with GDPR and confidentiality policy. Engagement in collaborative service delivery with colleagues across the services. Demonstrate knowledge of community services and resources.



Establish and maintains networking with GPS, commissioners, community mental health teams and Social workers. Planning and Organisational Skills: Ability to manage a caseload of young people and young adults with varied psychiatric presentations who may fluctuate in severity and complexity. Ability to manage their own time and prioritise their caseload and workload effectively, support the reallocation of workload when other staff are absent or on leave. Ability to meet deadlines for reports, liaison with external agencies and statutory meetings.



Ability to chair CPA meetings, LAC reviews and promoting young peoples and carers participation. Continuous Professional Development: Maintain RCN registration Participate in annual appraisals and Revalidation Take responsibility for keeping p to date with evidence based treatments and mandatory organisational training. Evaluate the effectiveness of their own nursing care through participation in supervisions, reflective supervisions and training. Benefits Discretionary annual bonus scheme.
