Job Description

We are looking for an enthusiastic Registered Nurse/ Clinical Lead to join our care team and provide outstanding quality of care to our residents in a friendly, homely environment.

St Michaels is a nursing care home registered for 36 residents.

Job Purpose



To assist the home manager in planning and implementing the highest quality of nursing care to a wide variety of home service users, some of which may live with dementia.

Objectives



Working closely with the Home Manager, the Clinical Lead will take the lead role in ensuring our service users receive the clinical care they need, in a way that supports dignity and independence.

As a Clinical Lead you will:

1. Ensure that there is effective commissioning of well-being of service users, with a smooth and efficient interface between our primary care and all other providers of care, including social care.

2. Ensure that safe and effective care pathways are in line with current policies and procedures within the nursing home.

3. Identify and support the development of relevant integrated care pathway protocols and processes in the nursing home, including: training; development; and mentoring of staff.

4. Identify gaps in services within the nursing home and develop options and plans to close these gaps.

5. Manage the nursing and healthcare teams, providing support, development, and leadership.

6. To ensure care plans and risk assessments are regularly reviewed and updated.

7. Respond to incidents in a professional and appropriate manner.

8. Establish and maintain relationships with service users, visitors, and employees.

9. Co-ordinate care in line with best guidance practices.

10. Ensure medicine management processes are followed and reviewed.

11. Provide clinical support to, and assessment of, service users' health needs.

Responsibilities and Duties



1. To provide a high standard of direct nursing care to service users based on the assessment of care needs and in consultation with the service users, their relatives, and the care team.

2. To continuously evaluate the quality of care given, and regularly reassess the needs of service users in consultation with the service users, their relatives, and the care team. Furthermore, to effect changes required to achieve the planned goals.

3. To ensure service user records are accurately maintained, including: daily record sheets; care plans; GP visits; and monthly observations. Any changes to care, condition or treatment must be recorded in the appropriate document at the time by the staff nurse caring for the service user.

4. To demonstrate effective oral and written communication skills, and to respect confidentiality.

5. To dispense and administer prescribed medications, and to order medication as required in accordance with NMC guidelines on medication.

6. To consult with GPs, Social Services, the Chiropodist, Occupational Therapists, and other members of the multi-disciplinary care team.

7. To work within the NMC codes of conduct and Scope of Practice, and within the home's policies and procedures.

8. To continuously review their own nursing practices to develop new skills and knowledge through continuous professional development training. This training will contribute to the enhancement of patient care.

9. To work closely with other members of the care team, ensuring that effective, high-quality care is given, to achieve planned goals.

10. Monitor any documents that demonstrate care issues, e.g. fluid/weight/wound charts.

11. To consult with other members of staff, to ensure the home is kept clean, tidy and in good repair.

12. To consult with the cooks regarding changes to dietary needs of service users.

13. To ensure the effective and efficient use of all home resources, reporting any faults to facilitate repair and replacement.

14. To keep next of kin informed of significant changes in a service user's condition, especially notification of all accidents and GP visits.

15. To ensure religious, cultural, emotional, and psychological support for service users, and their families, are met.

16. To attend and participate in relevant staff meetings.

17. To ensure service users' valuables are handled and recorded appropriately. Two signatures required.

18. To undertake supervisions and appraisals with the Manager.

19. Support the Home Manager and registered providers in their duties as the "Person Responsible" for the home under the Health and Safety at Work Act 1974, and the Fire Regulations and the Health and Social Care Act - mainly Reg 16 and Reg 18.

20. To be aware of the locations of mains gas/water/electric - including trip switches.

21. To be aware of action to take when there is an admission to the home, or in the event of a death. i.e. the records/procedures that must be followed and maintained, including Admission and Discharge Book.

22. To give Quality Questionnaire surveys to relatives on departure of residents.

23. To inform the Manager of all incidents/deaths where a CQC notification is to be raised.

Principle Purposes



1. To ensure that the team deliver quality care according to the operational policies and procedures of the home.

2. To review, audit and implement evidence-based care planning and delivery.

3. To maintain a safe environment for residents, families, visitors, and staff.

4. To ensure that carers and support staff are well inducted, and to appraise carers and maintain their personal development plans.

5. To ensure effective communication channels between staff, management, residents, relatives, significant others, and visitors.

Working Relationship



1. To supervise the work of all staff within the team.

2. To work proactively with 'Key Workers.'

3. To support the senior management team of the Home.

4. To deputise, in the absence of the Home Manager.

5. The notice period for this role is at least one calendar month
