Job Description

We are looking to recruit a Band 5 Registered Nurse to join our team on Ward 2, Dewsbury & District Hospital. Ward 2 is a 33 bedded unit, caring for elderly patients who no longer have a medical Reason to Reside in hospital, and are awaiting discharge planning before leaving hospital. We are looking for a highly motivated registered nurse with a passion for providing high standards of care and patient experience.
Main duties of the job
The primary aim of the role is to provide effective patient centred care by performing individual assessment of care needs, developing, implementing and evaluating programmes of care that meet the needs of the patient and their families/carers. Manage and ensure discharge planning is progressing, by working with the discharge, therapy and social worker teams to prevent delays in discharge, and ensure patients are being discharged to the most appropriate place for their needs.
