We are seeking a compassionate and dedicated Registered Nurse and Infection Control Lead to join our growing company Golden Community. The Registered Nurse (RNA or RMN) will provide comprehensive nursing care to patients in our newly developed 6 bed Nursing home for complex chronic diseases, dementia, palliative and End of Life residents. The RN will be responsible for assessing patient needs and delivering high standards of personal care, developing care plans to support their personalised support plans for health, social and wellbeing, administering treatments and medications, and collaborating with other healthcare professionals to ensure high-quality patient care at all times. This is a unique opportunity to work in clients own home and in a small nursing home in a quaint village, with beautiful surroundings, ensuring residents receive the nursing care and support they deserve. The Nursing home will also provide day care, activities and wellbeing, such as regular health promotion and screenings, encouraging the community to be part of the Nursing support network.
Key Responsibilities:
Patient Assessment and Care Planning:
o Conduct thorough assessments of patients' health conditions.
o Develop and implement individualised care plans based on patient needs.
o Monitor and evaluate patient progress, adjusting care plans as necessary.
Direct Patient Care:
o Administer medications and treatments as prescribed by healthcare professions.
o Provide wound care, catheter care, venepunture and other nursing procedures.
o Assist with activities of daily living (ADLs) and provide support to patients and their families.
Coordination and Collaboration:
o Coordinate with other healthcare professionals, including GPs, therapists, and social workers.
o Communicate effectively with patients, families, and caregivers to ensure continuity of care.
o Participate in multidisciplinary team meetings to discuss patient care plans.
Documentation and Reporting:
o Maintain accurate and up-to-date patient records.
o Document all nursing interventions and patient responses.
o Prepare and submit reports as required by regulatory bodies and the healthcare facility.
Education and Support:
o Educate patients and their families about health conditions, treatments, and preventive care.
o Provide emotional support and counseling to patients and their families.
o Train and supervise care aides and other support staff.
Compliance and Safety:
o Ensure compliance with healthcare regulations and standards, including CQC, local and national standards.
o Follow infection control protocols and maintain a safe environment for patients and staff.
o Participate in quality improvement initiatives and continuing education programs.
Qualifications:
Education:
o Registered Nurse with the NMC.
o Evidence of Revalidation and CPD
o Training and experience in Infection Control
Experience:
o Minimum of 1-2 years of nursing experience, preferably within a care home or community care settings.
o Experience with electronic health records (EHR) systems is a plus.
o Experience with leading infection control meetings.
Skills:
o Strong clinical and critical thinking skills.
o Excellent communication and interpersonal skills.
o Ability to work independently and as part of a team.
o Proficiency in using medical equipment and technology.
Other Requirements:
o Valid driver's license and reliable transportation for community visits.
o Current enhanced DBS.
Working Conditions:
The RN will work in various settings, including our care homes and patients' own homes.
The role may require travel within the community and flexible working hours, including evenings and weekends.
