We are seeking a compassionate and dedicated Physical Health Nurse to join our healthcare team, working in one of our Mental Health Clients down in Portsmouth.
The ideal candidate will possess a strong commitment to providing high-quality patient care and will be responsible for assessing, planning, implementing, and evaluating patient care plans. This role is essential in ensuring the well-being of patients and supporting their recovery journey. We require the Nurse to be competent in taking bloods, doing physical exams and have tissue viability experience.
Duties
Conduct thorough assessments of patients' health status and needs.
Develop and implement individualised care plans based on patient assessments.
Administer medications and treatments as prescribed by physicians.
Monitor patients' progress and respond to changes in their condition.
Provide education and support to patients and their families regarding health management.
Collaborate with multidisciplinary teams to ensure comprehensive patient care.
Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
Uphold infection control protocols and maintain a safe environment for patients.
Skills
Proficient in providing exceptional patient care with a focus on empathy and support.
Strong knowledge of anatomy to effectively assess and understand patient conditions.
Excellent communication skills, both verbal and written, to interact with patients, families, and healthcare professionals.
Ability to work well under pressure in fast-paced environments while maintaining attention to detail.
Strong organisational skills to manage multiple tasks efficiently.
Commitment to continuous professional development and staying updated on best practices in nursing.
