Practice Nurse for immediate start

The team at Wellington House Surgery are looking to recruit an enthusiastic Practice Nurse to work with us in our rapidly developing practice.

Our Practice -

Situated in the up-and-coming Yorkshire town of Batley, close to the M62 / M1 Motorways. We have a list size of 9,000 patients with a varied patient demographic. We are a GMS, SystmOne Practice.

Our Team -

8 GPs, 1 ACP, 2tACP, 1 Practice Nurse, 1 Nurse assosicate and 2 HCAs supported by a friendly and efficient admin team.

We are currently enrolled in the tACP mentoring programme with a forward view of expanding into further areas of training and mentoring of GPs, PNs & NA.

Job Plan -

To support patients to improve and maintain their mental, physical, behavioural health and well being by using behaviour change interventions, such as undertaking health checks (physical and mental). To support health action planning, as part of Learning Disability and Severe Mental Illness physical health checks, with the support of practice clinical leads and community specialist teams.

To undertake Chronic disease management (e.g. asthma, diabetes, COPD). Immunisations and vaccinations (including travel vaccines). Cervical cytology (smears). Complex wound care and dressings, Health promotion and lifestyle advice

To maintain effective infection control measures in line with local guidelines and practice policy. To participate in developments within the practice / network and support QOF / DES requirements.

To be registered with the NMC. Work in accordance with the NMC Code of Conduct and Scope of Practice for Practice Nurses Ensure accurate notes of all consultations and treatments are recorded in the patients notes and on computers, as per NMC guidelines.

Qualifications

NMC-registered Practice Nurse with recent primary care experience Confident in managing long-term conditions and working independently Skilled in using clinical systems ( SYSTMONE- desirable but not essential) Compassionate, organised, and a strong team player

Job Type

- Practice Nurse, Part time - 18-25 hours per week.

The starting rate is negotiable depending on experience and will be discussed at the interview.

Start date flexible and to be agreed.

Job Types: Part-time, Permanent

Pay: 21.00-23.00 per hour
