Job Description

The successful candidate will join our experienced multidisciplinary team of skilled and highly motivated staff in our 16-bedded inpatient community ward. We deliver quality services for patients requiring generic rehabilitation for older persons, deliver frailty, subacute and discharge to assess pathways . This opportunity will allow you to experience a holistic view of community services and gain knowledge of the full range of health and social services available for patients, both within a hospital environment and other services in the community that can help the patient stay in their preferred discharge destinations safely.

An opportunity for leadership and clinical supervision is provided to all ward staff, enabling the maintenance of professional standards within NMC guidelines to provide high standard of patient care. You will work alongside a multi-disciplinary team of Doctors, ANPs, Therapists, Nursing staff, Pharmacists and support workers to provide high quality care to patients on our ward.



To promote and maintain a high standard of individualised care by actively contributing to the planning, delivery, evaluation of care, ensuring that care plans are used effectively, adhered to and updated as the patient's condition dictates.



To maintain a safe, welcoming environment for patients, relatives, visitors, and staff.



To demonstrate clinical decision-making skills, displaying evidence-based practice



To deliver patient care using a variety of clinical skills in line with competency framework.



To be actively involved in promoting self-care for patients and building health promotion into care as appropriate .

The "must haves" for you to be considered for this role:



Registered Nurse /Adult Nurse Trained along with complete NMC registration



To work bank holidays/weekends or unsocial hours to meet the needs of the service



Reasonable level of fitness required



Good Communicational and Interpersonal Skills
