The Discharge to Assess (D2A) Team has recently merged with our Bed Based teams across our 3 Community Hospitals. This is a hybrid role working as part of the D2A team but embedded within the inpatient team at Herts & Essex Hospital (HEH). You would be joining a supportive team of qualified therapists and therapy support workers to provide quality care to our D2A patients within care homes in and around the East & North Hertfordshire area, as well as the inpatients at HEH.You will be based at HEH in Bishop's Stortford, Hertfordshire. However, when providing the D2A service, the post holder will be required to travel between care homes within East & North Hertfordshire including, Stevenage, Hatfield & Broxbourne.HEH is a 32 bedded unit located in Bishop's Stortford, Hertfordshire. HEH provides rehabilitation for patients with a wide range of conditions such as falls, frailty, orthopaedics and amputees. Rehabilitation is provided on a generic 17-day pathway.Part time hours between 22.5-30 will be consideredMain duties of the jobTo assist qualified allied health professionals, including occupational therapists and physiotherapists in the delivery of rehabilitation in a variety of settings.To work without direct supervision, but under the direction of the qualified staff in the performance of specific tasks for an individual or group, as part of their treatment and rehabilitation programmes. This may be in a community setting such as the patient's own home.
