An exciting opportunity has arisen for an enthusiastic and motivated individual to join our Community Therapy Team as a Rehabilitation Therapy Assistant .
As a vital part of our multi-disciplinary team, you will contribute to delivering a patient-centred rehabilitation service, working closely with occupational therapists, physiotherapists, and other healthcare professionals.

Key Responsibilities
? Deliver rehabilitation through therapy interventions , which may includ e support with personal care tasks, cogniti on and mobility
? Carry out rehabilitation plans as d elegated by occupational therapists and physiotherapists.
? Maintain accurate documentation and communicate effectively with service users, families, and the therapy team.
? Be flexible to support the team during peak service times.
? Participate in the 7-day service rota, including weekends , delivering rehabilitation between 8am and 8pm.
About You
Passionate about patient care and rehabilitation.
Excellent communication and teamwork skills.
Ability to work independently and as part of a multi-disciplinary team.
Organise d with good tim e management skills.
E xperienc e in a healthcare or therapy setting is desirable but not essential.
