Job Description

Advanced level practice encompasses aspects of education, research and

management but is firmly grounded in direct care provision or clinical work with

patients, families and populations. Nurses working at an advanced level promote

public health and well-being. They understand the implications of the social,

economic and political context of healthcare. Their expertise, experience and

professional and clinical judgement are demonstrated in the expert nature of their

practice and the depth of their knowledge.



To be the focal person within the specialist pathway of care, ensuring that patients

and their families/carers are supported and kept informed throughout the journey

from initial investigations, through to diagnosis, treatment and on into follow up

care.

To exercise a pivotal role in the liaison between primary, secondary and tertiary

care and between colleagues within the team; ensuring that the service is

underpinned by timely and accurate communication of information and is focused

on the needs of each individual patient.

To provide expert clinical case management to a specified group of patients their

families and communities within a defined population.

To undertake advanced level holistic assessment in order to address complex

health care needs by using expert knowledge and clinical judgment to diagnose,

initiating interventions and refer appropriately.

To strategically lead the co-ordination of care, working with partner agencies and

key stake holders to ensure effectiveness of outcomes for defined case mix of

patients.

To ensure that services are delivered effectively and efficiently against

predetermined clinical performance targets.

Have a proactive approach to promoting health and wellbeing, detection and

prevention of vulnerability, preventing illness, reacting and responding to existing

or potential ill health issues and contributing to reduction in length of stay or

reducing unplanned hospital admissions.



Conduct yourself and practise within an ethical framework based upon respect

for the well-being and safety of patients and clients.

The post holder will be accountable and responsible for the clinical case

management of a defined population.

The post-holder will initiate and lead the delivery of planned programmes of care

to individuals, families and communities in a variety of settings as part of a multidisciplinary/

multi-agency team.

The post-holder will collaborate with a wide range of statutory and voluntary

organisations to maximise the nursing contribution in health improvement,

planning and delivery of care to individuals, families and communities.
