This post in Renal Transplant has arisen at a very exciting time in our renal service at Bradford as we embark on service expansion involving state of the art build work to our main sites and commence service relocation and configuration options for our satellite unit at Skipton.



Bradford is a rapidly developing diverse and multicultural city with easy access to the major cities of Leeds and Manchester and close proximity to the Yorkshire Dales. Renal services in Bradford are provided by a multidisciplinary teams of dedicated nephrologists, renal specialist nurses, advanced kidney disease, dialysis, dialysis access, dialysis education and renal transplantation) and in close collaboration with multi-agency teams as diabetes, dietitians, pharmacists, psychologists, dialysis technicians, research nurses, a clinical audit officer and a cultural and health improvement officer.



We are looking for an enthusiastic and motivated band 6 RGN, with significant experience in Nephrology / Transplantation nursing background, to join our transplant team. You will be based within the Renal Ambulatory Care Unit in the Horton Wing of St Luke's Hospital in Bradford.



We are looking for a dedicated nurse to co-ordinate and organise all aspects of kidney transplant care and care provision, working in collaboration with the multi-disciplinary team across the local and regional transplant unit to ensure all relevant clinical care for kidney transplant patients.



The successful applicant will need to be able to drive and have a current driving license.



You will possess excellent communication, problem solving and time management skills. You will be expected to maintain awareness of relevant research, evaluations and audit, and act on this information in collaboration with colleagues to continually develop the service. The post-holder will be responsible for the management of a defined caseload kidney transplant patients and will provide specialist nursing support, advice, education and/or care to this specific group. They will carry continuing responsibility for the assessment of needs, the development, implementation and evaluation of programmes of care and the setting of standards within the Renal Transplant Department.



This will include:



Supporting the repatriation of newly transplanted patients.

Providing transitional care support for young adult transplant patients in collaboration with the paediatric transplant team in Leeds.

Providing patient and carers with support, advice and education, including participation in a transplant patient forum and the appropriate use of social media.

Supporting the renal anaemia programme, including a drop in clinic.

Co-ordinating and monitoring of blood test results, including clear communication with the Transplant Immunology and Virology laboratories in Leeds.

Overseeing of post-transplant clinics.

Involvement and delivery of transplant anniversary reviews.

Working in conjunction with all relevant professional groups, including the advanced kidney care team, renal dietitians, renal pharmacists, renal psychologists, a cultural and health improvement officer, diabetes specialist nurses, allied specialties including radiology and urology, and primary care teams (using the SystmOne Renal Unit Hub).

Support for transplant audit, quality improvement and research.
