Who are we looking for?
An exciting opportunity has arisen for a Resettlement Broker to join us with a contract that has been commissioned as part of the resettlement provision attached to the wider healthcare contract, working closely with HMP Wandsworth. In this role you will work with service users with acute mental health concerns, in custody and through the gate into the community. Whilst developing and delivering healthcare resettlement services both in custody and the community, this will range from the delivery of in custody groups, to collecting service users on the day of release and providing ongoing support to them in the community.
Duties & responsibilities include but are not limited to:
Carry out initial assessment of the potential or actual service user, including any risks, or his needs and requirements in respect of the service on offer.
Design and deliver an effective programme and support plan with the service user using specialist agencies as applicable.
Maintain contact on a regular basis with service users in accordance with the requirements of the service. This includes maintaining a professional relationship and being a good role model to service users.
Risk assess and take the necessary actions in respect of any activities / engagement with service users including personal safety.
Develop relationships / liaise with appropriate external groups / agencies in support of services users' needs and requirements.
Work collaboratively in a multi-agency setting as required. This includes maintaining professional relationships and appropriate confidentiality of information.
Maintain up to date records and produce relevant reports as required.
The successful candidate will have an understanding or experience of providing information, advice and guidance to offenders or ex-offenders in line with resettlement needs. Knowledge of the Welfare benefits system and Homelessness legislation with reference to the priorities and entitlements of prisoners and those released from custody.
A DBS check will be required for this role
