This role would suit an Allied Health Care Professional, especially a Paramedic, Physiotherapist, or a Specialist Nurse. The successful candidate will play an integral part in assessing, supporting, and treating people with a range of respiratory conditions caring for and improving the standard of care for respiratory patients.
The successful candidate will have the opportunity to work within a variety of settings. The award-winning team is based in the Chest Clinic at King Edward VII Hospital in Windsor, but we see patients as hospital in-patients, in clinic and in their own homes. The focus is supporting people to manage their condition within the most appropriate setting. No two days are the same.
Having launched in October 2016 we are now expanding the team so the AIR service will provide respiratory care across East Berkshire in and out of the hospital setting, delivering acute respiratory care for patients with Asthma, COPD, Home Oxygen, Interstitial Lung Disease, Bronchiectasis, Respiratory Physiotherapy and Pulmonary Rehabilitation.
The role is a predominantly 0.8 WTE Monday - Friday with the hours being 9-5, to be agreed. There is some requirement to work 8-4 at times and the potential for an occasional Saturday rotation.
Assessing and reviewing respiratory patients at home as part of the Supportive discharge, admission prevention or acute respiratory virtual ward.
Visit patients in their homes to assess their progress, using advanced assessment skills; plan, deliver and evaluate patient care during their period of acute illness, and have advanced understanding of the complexity of issues surrounding the patient and their care
Using effective counselling/educational skills which may involve breaking bad news on diagnosis or prognosis of a chronic condition.
To contribute to palliative care of respiratory patients as they approach end of life.
Assessing and reviewing patients who are on long term oxygen therapy within their usual place of residence
To provide oxygen assessment to those patients referred to the service within the clinic setting, considering the most appropriate modality should oxygen be required.
Assess acutely unwell patients with respiratory illness in particular COPD who have been admitted to the Acute Hospitals (WPH) with the view to supporting discharge. This will involve reviews of their physical needs, their social and psychological circumstances and will require advanced assessment skills in those areas
