We are a Nursing & Residential Care Home that provides many health care services including palliative care, dementia care, respite care, as well as every day care for adults. We currently have one position available for 28 hours per week and pay will be reflected on experience.

Some Key Duties and Responsibilities



Managing Medication - In addition to administering medication, an RGN/RMN is responsible for ordering, receiving and auditing all medication that is stored at the care home. The return of any unused medication is also completed by the nurse in charge. The role is required to check medication quantities and ensure that the administration of all medication is properly recorded as stated in the guidelines set by the NMC and NICE; and to make arrangements and support the review/re-assessment of medication with the resident's GP.

Completing Assessments and Personalised Care Plans - Registered Nurses work closely with each resident and the care team to implement care plans that reflect the best interests and choices of the resident. Assessments highlight the needs and risks faced by the resident which are all considered in the care planning process. This role is responsible for monitoring the care of residents in areas such as mobility, food and fluid intake, level of dependency and ability to maintain safety, just to name a few. These person-centred risk assessments and care plans are regularly reviewed by the nursing and care team, as part of the ongoing care process.

Leading a Care Team - The on-duty nurse is responsible for ensuring the care team actively works together whilst supporting the residents. It is important to us that the nurses have a priority to deliver a high standard of care and to ensure that the care staff work to the same standards, using a mentoring and coaching approach. The Nurses along with Managers and Senior care staff lead the staff to ensure all staff adhere to the policies and procedures of the care home and complete duties effectively according to the NMC. As a Registered Nurse based within a care setting, the post holder needs to be a good decision-maker, as it is often necessary to allocate staff to different areas of the home, as well as delegate duties amongst the care staff.

Coordinating Multi-Disciplinary Teams - The role requires liaison with many professionals within the Health and Social care sector. From registering new residents with the local GP, to making referrals, to making various appointments and referrals to health care specialists such as psychiatrists it is the responsibility of the nurses to have effective communications with all team members involved in the care of the residents. It is vital to maintain a strong relationship with residents and have a keen clinical eye in order to notice any changes in the residents' health, to enable early identification and referrals as required.

Communication and Record-Keeping - Excellent communication and documenting skills are necessary for all aspects of delivering care and the well-being of others. Being able to communicate with residents, staff, family/visitors and other professionals and having an approachable, respectful manner, is vital for this role. Staff are responsible for accurate record-keeping, either digitally or manually using paper-based systems, in a prompt and appropriate manner. At the beginning and end of every shift, handovers are conducted between the nurse and care staff. All changes and updates to monitoring forms, care plans and risk assessments etc are conducted by the nurses.
