We are Trafford Local Care Organisation - a public sector organisation bringing together NHS community health services and adult social care services in Trafford. We're driven by the aim of improving the health of local people in Trafford by working across organisational boundaries.Our community healthcare teams inadults andchildren's servicesare employed by Manchester University NHS Foundation Trust and deployed to us, linking closely with our colleagues in Manchester Local Care Organisation. Our teams work as one and together we will make a massive difference to the health of people in Trafford.These posts present an exciting opportunity to work with families and young children to improve health and social outcomes and make a real difference in a true multi-agency environment. You will also need to demonstrate a positive attitude, flexibility and the ability to communicate well.Applications are welcome experienced School Nurses who are proactive and enthusiastic with excellent communication, interpersonal and organisational skills. A willingness and ability to contribute to, and lead changes within children's services are essential to the role.Main duties of the jobThe post holder(s) will work as part of the School Health team to promote and maintain the health and well-being of children, young people and families. This is an integrated role within the TLCO that will provide services in a variety of community settings such as clinics, GP surgeries, schools, Children's Centres and clients' own homes.The post holders will work as part of a School Health team with responsibility for the implementation of the Healthy Child Programme 5-19 years, the assessment of health care needs of children, young people and their families. The post holder(s) will work within the team to promote and improve health and reduce health inequalities; this will be underpinned by:

  • Identification and addressing of the health needs of children, young people, and families
  • Stimulate the awareness of health needs
  • Facilitate health-enhancing activities
  • Influence policy affecting health.
The post holders will promote the safeguarding of children, working within a multi-agency team.There will be a responsibility for the post holders to ensure named staff members have an annual Appraisal and Development Plan.Whilst successful applicants will primarily work within one team, all applicants may be required to work at other locations to ensure equitable service delivery.
