Sefton Council's Adult Social Care Occupational Therapy & Sensory Team is seeking seven experienced Occupational Therapists to join our service on an agency basis.
These roles have been established through Extra Care Fund (ECF) capital funding to support key priorities within the borough - focusing on promoting independence, supporting hospital discharges, and improving outcomes for adults with physical, functional, and sensory needs.
Key Responsibilities
Undertake functional assessments to identify and address barriers to independence within the home and community.
Recommend and facilitate the provision of equipment and major adaptations, working in line with relevant legislation and local policies.
Support safe and timely hospital discharges, collaborating closely with health and social care partners.
Contribute to the prevention of hospital admissions through proactive intervention and rehabilitation planning.
Maintain accurate case records, reports, and documentation in line with professional and organisational standards.
Requirements
Degree or Diploma in Occupational Therapy.
Current registration with the Health and Care Professions Council (HCPC).
Demonstrable experience within adult social care, hospital discharge, or community-based OT services.
Strong assessment, communication, and problem-solving skills.
Ability to work independently and as part of a multi-disciplinary team.
Enhanced DBS required.
