The inspection highlighted that students with high needs make very good progress while at college and our approach is to support the students to increase their independence and develop the skills and confidence to be successful in their everyday lives and their next steps be that higher education or employment.
We are seeking a passionate and experienced SEND Transition Coordinator to join our dynamic study centre team and help facilitate a seamless transition process for these students into post 16 education. As the SEND Transition Coordinator, you will play a pivotal role in supporting students with SEND as they transition from one educational phase to another. You will work closely with our partner secondary schools as well as students, families, and other key college staff to ensure a smooth and supportive transition process that addresses individual needs and promotes successful outcomes.
Key Responsibilities will include:
Transition Planning: Develop and implement individualised transition plans for students with SEND, ensuring they are tailored to each student's unique needs and aspirations.
Collaboration: Work closely with students both prospective and enrolled, SEND staff in partner schools, external agencies where appropriate, and families to coordinate and facilitate transition activities and support.
Support Services: Provide guidance and support to students and families during the transition process, including organizing and leading workshops and information sessions.
Monitoring Progress: Track and evaluate the progress of students during and after the transition period, making adjustments to plans as necessary to ensure continued success.
Documentation: Maintain accurate and up-to-date records of transition plans, meetings, and progress reports.
The successful candidate should be educated to degree level and have a good understanding of SEND and High Needs students regulations. They will have excellent communication and organisational skills, patience, a sense of humour and be able to work under pressure. They need to be able to work independently as well as part of a team.
This is a full time, term time only role, returning from the summer break on GCSE results day. There will also be additional 10 plus days for workshops with parents and students during the holidays as agreed with the College. All year round may be considered for an exceptional candidate.
The successful candidate will be required to undertake an enhanced DBS check. Please be aware that we are also required to conduct online searches as part of our safeguarding checks and due diligence for all shortlisted candidates.
