Working in close collaboration with social care (KCC), community health teams, and acute hospitals, Home First supports patients during transition periods by offering enablement-focused care.
The service aligns with national integrated care strategies and is underpinned by a person-centred approach, empowering individuals to achieve outcomes that promote recovery, autonomy, and well-being in their home environments.
The Senior Assessor plays a pivotal role in delivering high-quality, person-centred care within the Home First service.
They provide advanced assessment, complex case management, and operational support, while also guiding and developing colleagues to maintain excellence across the team.
