Duties:
Supporting Team Leaders to assess, plan, implement, and evaluate the comprehensive care needs of service users.
Delivering hands on personal care and support to provide the highest standards of person-centred care in the community.
Prompting medication in line with the service user's care plan
Maintain accurate records of the job you do which will include completing assessments, writing care plans, reviews, and evaluations.
Ensure any issues relating to a service user's health and wellbeing are dealt with effectively and/or escalated to senior management
Provide personal care and support to individuals in their own homes or in a care facility
Assist with activities of daily living, including bathing, dressing, and toileting
Prepare and serve meals according to dietary needs and restrictions
Monitor and report any changes in clients' physical or mental health status
Skills:
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational skills and attention to detail
- Driving licence
