An excellent opportunity has arisen within Diamond Home Care for the role of Senior Care Worker for the Sandwell Brough

The successful candidate will be based from our head office at 1st Floor, Old School House, Church Road, Netherton, West Midlands, DY2 0LY.

The successful candidate will be required to:-

- Work set shifts in the community as well as supervising staff and liaising with office staff and management

- Carry out field observations on care staff and assist with assessments and care plans- To ensure high quality care is delivered to clients in their own home

- Communicate effectively with staff, clients and ensure all records are up to date

- Participate in the out of hours rota

To be successful, you will need to have:

Previous care experience
Good communication skills; written and verbal
Good planning and organisational skills and work on own initiative
An understanding of good care principles and regulations
IT literate
Be aware of local care companies advertising higher rates of pay but not including a separate payment for the time that it takes for you to travel in between care visits.

Higher rate of pay without travel time can result in you earning under the minimum wage (more information on our pay structure will be sent to you before your interview)

Full time post with guaranteed contracted hours and paid travel time. Excellent opportunities to further career and gain valuable experience.

Purpose of post:

As a Senior Care Worker, you will be responsible for overseeing and providing high quality care and support to customers. Your role will involve supervising and guiding a team of care workers, ensuring the delivery of person-centered care plans, and maintaining the overall well-being of our customers. Your role will entail you meeting any deadlines that are set by your line manager in a timely manner. This position required strong leadership skills, compassion, and a deep understanding of the unique needs of older adults. You will support the Care Co-Ordinator to implement Diamond Home Care's Operational and Supervisory Management Responsibilities and Key Performance Indicators to enable you to effectively deliver your Senior duties.Care Plan Reviews & Assessments, Delivery of New Packages of Care, Management of Shadow Diaries, Service User Reviews, Spot Checks, Medication Audits, Liaise with Other Care Professionals,

You will adhere to senior management instructions and promote the service in a positive manner to all relevant stakeholders.

You will recognise your professional boundaries and work within these i.e., deliver a friendly and professional service, getting to know your customers' needs but without crossing professional boundaries.

Principle Tasks:

1.1 People Planner and Access Care Planning

You will conduct and devise person centered care plans on access care planning within the contractual guideline. You will contact customers and/or representative and book assessment visit within (1hr) and conduct home/hospital visit within (24hrs) to carry out full assessment and ensure a schedule of reviews are undertaken at six- or twelve-monthly intervals.

Person Centered Care Plans should include as a minimum a person's strengths & assets, how they will regain/increase/maintain confidence in their independence; personal details, key contacts, friends & family, inter-agencies, health history, religion, sexual orientation, hobbies & activities, medication, mental capacity, visit schedule & care tasks, risk assessments.

1.2 Mentoring

To lead by example and promote Diamond Home Care's values and responsibilities to care workers by organising, supporting and maintaining the Shadow Diary & Integration of new Community Staff by providing detailed customer care plans and appropriate on-the-job support to ensure that they can implement the 15 Units of the Care Certificate Induction Program into their delivery of tasks within the packages of care.

1.3 Business Growth

Assist in business growth and development of services by completing assessments and visits of new packages of care until such time that they are fully assessed and handed over to a structured round and templated to a Community Care Worker.

1.4 Key Worker Support

To support the care co-ordinator to identify and facilitate any carer key workers. The support key workers to report changes of circumstances of customers needs transparently.

To cover scheduled holiday requests and absences cover, and to prevent any possible missed calls within your service area, by covering calls at very short notice to minimise complaints and/or concerns.

1.5 Out of Hours

To participation in the Rota of the Out of Hours Service, between all the Seniors within your area of responsibility. Ensuring that all records are maintained and completed contemporaneously, (People Planner - Alerts & Tasks and OOH Daily Logs) ensuring the you bring to the attention of your Line Manager any Medication Errors, Incidents or Accidents.

1.6 Professionalism

To promote a professional image and cultivate an open, transparent, and supportive working environment. Support the building of relationships on behalf of Diamond Home Care with local authority brokerage and other relevant teams, by providing statistical data pertaining to employee availability and surplus service capacity. Also ensuring that any temporary or short-term packages are extended to full- time provision.

1.7 Rostering

To support the Care Co-Ordinator to create and maintain staff schedules using People Planner, by providing up-to-date and concise information pertaining to staff absences and management of staff holiday requests.

1.8 Quality Assurance

- To conduct quality assurance visits with customers via home visits to complete the Service Users Reviews. Action all feedback in a swift, effective, and in a professional manner, taking guidance from your Care Co-Ordinator.

- To complete Meeting Record(s) with Community Care Staff, Inter-Agencies, Family & Friends and/or Contracting organisations regarding any queries or incidents that are not outlined within the People Planner Rostering Management Software.

- To participate and co-ordinate Quality Assurance Audits (Staff Spot Checks & Medication Audits) within your areas of responsibility on the Community Care Staff in-line with the Care Quality Commission Regulatory requirements and internal Quality Assurance Auditing schedules.

1.9 Complaints

To support your Line Manager with the Complaints, Concerns, and Compliments procedures by participating in and/or carrying out investigations in the community.

2.0 Policies

To conform to all policies and procedures laid down by the organisation in respect of carrying out these duties and in other administrative aspects of the Business, as relevant.

3.0 Training

To participate as directed by the Supervisor/Manager in induction training and regular on-going training programs.

4.0 Record Keeping

To maintain accurate, concise and timely records of customer care including dietary, emotional, physical, wellbeing, medication and personal care tasks including any other tasks that may have been completed.

5.0 Supervision

- To participate in staff, team and quality management meetings as directed by you line manager or senior management team.

- To participate and attend to any supervisory/performance meetings.

6.0 Communication

- To provide effective communication to customers in their homes and following their life history as documented in their care plan.

- To report back to the supervisor/manager on any aspect of a customer's care/service which may warrant investigation or urgent action.

- To report any change in circumstances or safeguarding concerns.

- To report any absences in a timely manner in line with Diamond Home Care Absence Policy.

- To adhere to Diamond Home Care's communication boundaries policy.

7.0 Customer Engagement

To participate and complete in reviews of customers care plans as required.

8.0 Professional Boundaries

- To work within professional boundaries and not develop inappropriate relationships with customers that may present a conflict of interest in your duties under the role of Senior Care Worker.

- To adhere to Diamond Home Care's professional boundaries policy.

9.0 Personal Care

- Dressing/Undressing/Preparing the customer for day care or trips out.

- Washing/Bathing/Showering/Shaving/grooming/Cleaning teeth

- Hair Care (Washing/brushing)

- Toileting and all aspects of personal hygiene

- Continence management

- Care of pressure sores (under appropriate nursing supervision)

- Getting in and out of bed

- Assisting with the use of aids to daily living/rehabilitation aids, as required

- Helping with rehabilitation programs, as prescribed by healthcare professionals -Day/evening/night sitting services, as required.

Knowledge and Skills Required

ESSENTIAL

Possess a solid understanding of care principals, including personal care, hygiene, medication administration and safety protocols.

Be flexible and adaptable to meet the changing needs and routines of customers and company changes.

Full valid driving license and access to vehicle for work purposes (with business insurance cover)

Excellent written & verbal communication skills including a good telephone manner and the ability to keep accurate record keeping.

To have a good understanding of common age health related conditions such as dementia, mobility issues, chronic illnesses, and mental health concerns.

To hold a minimum of a NVQ Level 2 and have a willingness to actively participate in on-going training and refresher program including completion of Level 3 in Health & Social Care.

Possess strong leadership and supervisory skills to effectively guide and support a team of care workers.

Ability to maintain confidentiality in line with policy.

Demonstrate empathy, compassion and a genuine concern for the well being of customers.

Exhibit strong problem-solving skills to address unexpected situations, changes in customers conditions care needs. Provide creative solutions and seek assistance from healthcare professionals where necessary.

Efficiently manage time and priortise tasks to ensure all care responsibilities are fulfilled within designated time frames.

Show cultural sensitivity and respect for diverse backgrounds, beliefs, and practices. Understand and adapt care approaches to accommodate individual cultural preferences and values.

Possess strong documentation skills to maintain accurate and detailed records of care provided, observations made, any other relevant information.

Understand medication administration procedures, including dosing, timing, and potential side effects. Ensure accurate administration and adherence to prescribed medication regimens.

Familiarise yourself with the principals and practices of providing compassionate end of life care, including palliative care and support for both customers and their families.

Have the ability to assess customers needs and develop comprehensive care plans, and regularly review and update them based on changing circumstances.

To understand the role and responsibility of professional bodies i.e., CQC and to adhere to compliance tasks.

Stay updated with industry trends, best practices, and emerging research in care. Engage in professional development opportunities to enhance knowledge and skills.

DESIRED

Possess formal qualifications or certificates in the relevant field i.e. Level 3 in health and social care.

Specialist Care Training

Proficiency in multiple languages, especially those spoken by the local community.

Be an active listener, paying attention to customers concerns, preferences and feedback.

Possess knowledge and skills to support customers with cognitive decline or memory loss, such as dementia.

Working knowledge of the Domiciliary Care industry and surrounding regulations and legislation

Have prior experience working in a care related role, preferably in a similar care setting.
