Purpose of the Post:



1. To be aware and knowledgeable of the role and duties, as a post holder in keeping abreast to the philosophy of care.

2. To assist the Home Manager and Deputy Home Manager in the delivery of total patient care in meeting the physical and mental needs of clients to a high standard of care.

3. To carry out all duties in such manner as to enhance clients' quality of life in maintaining their independence, dignity, privacy, fulfilment of choice and their rights are not devalued.

Duties & Responsibilities:



Under the guidance of Senior Staff to carry out relevant duties befitting the post



1. Able to demonstrate and perform competencies of training record of care instructions and proficiency.

2. To act as team leader in achieving the necessary task for the welfare of clients within the existing policies of the Home.

3. Report to the Home Manager/Deputy Home Manager progress and condition of clients and other matters affecting their welfare as well as keeping adequate records in the care plan.

4. Contribute in assessment of clients care needs and participate remedial action as plan of care to members of the team.

5. To be ware of confidentiality with regards to clients' personal and medical conditions, and other ethical issues involved.

6. Effective use of communication skills and give reassurance to clients, relatives and visitors.

7. Assist in admission of clients / transfer/ escort appointments as directed by the nurse in change.

8. Assist in organising manpower and resources to provide to safe and high standard continuity of care.

9. To carry out policies required in respect of client's property and valuables.

10. To ensure clients' needs are met with regards to maintaining and promoting their activities of daily living - in a manner and act of excellence in the delivery of total client care.

11. Sufficient knowledge and understanding of precautionary and preventative measures of cross infection.

12. Awareness of knowledge based safe practices of moving and handling techniques as to minimise risks to clients and handler.

13. Assumes all reasonable precaution for a safe environment in accordance to Health and Safety Work Act.

14. Promptly report all hazards/defective equipment and other areas of concern including mishaps and incidents to the nurse in charge.

15. Able to act safety in an emergency of fire and has sound knowledge about procedures and prevention measures.

16. Attend training courses available to keep up knowledge and skills required for the post.

17. Participate in demonstration / orientation of new staff on training.
