Key Responsibilities
:
Service Delivery
:
Provide direct care and support to service users, including personal care, medication administration, meal preparation, and companionship.
Ensure that all care provided aligns with individual care plans and the unique needs of each service user, particularly for clients with conditions like dementia or physical disabilities.
Monitor and review the care and support needs of service users, updating care plans and risk assessments as necessary.
Supervision and Leadership
:
Supervise and support care workers in their duties, ensuring they adhere to the company's standards and policies.
Conduct regular spot checks, supervisions, and team meetings to ensure high-quality service delivery.
Mentor and coach staff, providing feedback and support for professional development.
Compliance and Documentation
:
Maintain accurate and up-to-date records, including care plans, risk assessments, and daily logs.
Ensure compliance with relevant health and safety policies, including the Lone Worker Policy, infection control, and safeguarding procedures.
Report any concerns related to the safety or well-being of service users promptly to management.
Client and Family Liaison
:
Build and maintain positive relationships with service users and their families, acting as a point of contact for queries or concerns.
Involve family members in care planning and reviews where appropriate, especially in cases involving cognitive decline or communication difficulties.
Risk Management
:
Conduct and update risk assessments, identifying potential hazards in service users' environments and health-related risks.
Use tools such as body maps to document any changes in service users' conditions, such as skin marks or bruises, to ensure proper care management.
Team Collaboration
:
Work closely with the management team to ensure that care services are delivered efficiently and in accordance with company policies.
Participate in team meetings, training sessions, and contribute to the ongoing improvement of the service.
Liase with local authorities in sourcing packages of care.
Attend collaborative meetings with local authories
Essential Skills and Qualifications
:
SVQ Level 3 in Health and Social Care
(or working towards).
Experience
in a senior care role, preferably in a home care setting.
Excellent communication skills
, both verbal and written, with an ability to engage effectively with service users, families, and staff.
Leadership and team management
experience, with a focus on motivating and supporting care staff.
Strong organizational skills
and attention to detail, particularly in managing care plans, risk assessments, and documentation.
Empathy, compassion, and patience

A good understanding of

Desirable Skills
