Job Overview


We are seeking a dedicated and compassionate Care Team Leader to oversee and support our night caregiving staff in providing exceptional care to our residents. The ideal candidate will possess a strong background in patient care, with experience in group homes, assisted living, or similar environments. As a Care Team Leader, you will play a crucial role in ensuring the well-being of our residents while fostering a positive and collaborative work environment.

Responsibilities



Supervise and support caregiving staff in delivering high-quality care to residents. Develop and implement individualized care plans tailored to each resident's needs. Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, and meal preparation. Monitor patients' vital signs and overall health status, reporting any changes to appropriate medical personnel. Ensure compliance with HIPAA regulations and maintain confidentiality of resident information. Provide training and guidance to staff on best practices for patient observation and care. Facilitate communication between residents, families, and healthcare providers to ensure continuity of care. Maintain cleanliness and organization within the living environment. Assist with medication administration as per care plans and regulatory guidelines. Engage residents in meaningful activities that promote social interaction and cognitive stimulation.

Skills



First aid and CPR certification is essential. Experience in group homes, assisted living facilities, nursing homes, or hospice care is highly desirable. Strong skills in patient observation, monitoring vital signs, and providing direct support to individuals with developmental disabilities or Alzheimer's disease. Familiarity with memory care practices and dementia care strategies. Ability to assist with heavy lifting using equipment such as Hoyer lifts when necessary. Proficient in meal preparation and cooking for diverse dietary needs. Knowledge of home health services and long-term care principles. Excellent interpersonal skills for working effectively with residents, families, and team members. Understanding of social work principles as they relate to caregiving.
